By checking this box, I agree that if I do not give a 2 week notice for end of services that I do owe the full month still and if I do not pay or my payment does not go through, I authorize to use the card below for that payment.*

I understand that this is an initial 3 month contract where I am committed to three months but then goes to a month to month basis with a 2 week notice for end of services. I know that I must give a 2 week notice before the payment date to end services or I will owe another month.*

By checking this box, I agree that if I do not give a 2 week notice for end of services after the initial 3 month period and at any point, that I do owe the full month still and if I do not pay or my payment does not go through, I authorize to use the card below for that payment. If the backup payment does not go through, I know I still owe for the month and will pay by another method.*
